Puerperal sepsis is a serious infection of the genital tract that occurs after childbirth, usually within the first six weeks postpartum. It is caused by bacteria entering the reproductive tract during or after delivery and is characterized by fever, lower abdominal pain, foul-smelling vaginal discharge, and general body weakness.

Background
Purity is the first-born child in a family of four and a Form Three student at a rural secondary school. She is active and sociable, and enjoys playing volleyball at school. Purity comes from a family with limited financial resources; her father runs a small-scale business that earns a monthly income of less than KES 1,500, which sustains the household. The family fetches their drinking water from a shallow well and lives in a two-room rental house. Despite these challenges, Purity is ambitious and dreams of becoming a nurse in the future. Three days after delivering her baby at home, Purity developed lower abdominal pain, fever, foul-smelling vaginal discharge, and back pain. Her condition progressively worsened, and her family was unable to afford medication or hospital care. Fortunately, during a community meeting conducted by a Helpster volunteer, Helpster Charity was highlighted as an organisation that supports vulnerable patients at Mechimeru Health Centre. After learning about Purity’s condition, a village elder visited her home and advised the family to take her to Mechimeru Health Centre, where Helpster Charity provides assistance to needy patients. Purity was diagnosed with puerperal sepsis.
Medical history
Purity was admitted to the hospital with puerperal sepsis and started promptly on broad-spectrum intravenous antibiotics to control the infection. She also received intravenous fluids, medication to manage pain and fever, and close monitoring of her vital signs and general condition. The medical team assessed her for possible underlying causes of the infection, including retained placental tissue or an infected wound, and provided appropriate management based on the findings. Purity responded well to treatment, with her fever and pain gradually subsiding and her overall condition improving. Once the infection was controlled and she was clinically stable, she was discharged with medication and advice on follow-up care, personal hygiene, adequate nutrition, and warning signs requiring immediate medical attention. With the timely treatment she received, Purity made a good recovery.
Prognosis
Following initiation of broad-spectrum antibiotics and supportive management, Purity is expected to demonstrate progressive clinical improvement. With adequate treatment and close monitoring, the risk of systemic spread of infection and other complications is minimized. Prognosis is good
