When a case is urgent and vital, hospitals often proceed with delivering medical care right away to save a life, even before the finalisation of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts. That’s why the chronology can sometimes be in disorder.

About chronic suppurative otitis media
Chronic suppurative otitis media (CSOM) is a persistent infection of the middle ear characterized by recurrent or continuous ear discharge through a perforated tympanic membrane. It commonly follows untreated or poorly treated acute otitis media, especially in children living in overcrowded or low-resource settings. CSOM leads to conductive hearing loss, delayed speech and learning difficulties, and may cause serious complications such as mastoiditis, facial nerve palsy, or intracranial infections. Early diagnosis, regular aural toileting, topical antibiotics, and timely surgical repair are essential to prevent long-term disability.

Background
Favour is an 11 yo child living with her parents, she attends a government secondary school, she wishes to be nurse in future. Her mother is doing her best with her petty trading to send her to school as the father is currently jobless. They live in a single room apartment, having a pit toilet, a well outside the compound and no access to electricity. Favour an 11yo girl presented with complaint of right ear discharge of one month following a preceding history of upper respiratory tract infection. There were occasional pains in the affected ear. No fever currently and no hearing impairment. On examination, patient is afebrile, not pale, not dehydrated, but in pains and discomfort. She was diagnosed with chronic suppurative otitis media and needs urgent treatment to reduce the pains and prevent further infection which might enter the brain and avoid hearing loss. The parents doesn't have the money to take her to the hospital, they reached out to Helpster Charity for help through a volunteer contact.
Medical history
Favour an 11yo female child presented with complaint of right ear discharge of one month following a preceding history of upper respiratory tract infection. There were occasional pains in the affected ear. No fever currently and no hearing impairment. On examination, patient is afebrile, not pale, not dehydrated, but in pains and discomfort. She was diagnosed with chronic suppurative otitis media. She was given adequate care on admission, treatment was given to stop ear discharge with otomed ear drop, some intravenous antibiotics, and ear spring. Patient did well and was discharged when ear discharge stopped and no more having pains and discomfort. Counselling was also given to patient’s parents to avoid water from entering the ear. They send their gratitude to Helpster Charity for the medical support.
Prognosis
Favour an 11-year-old girl is suffering from chronic suppurative otitis media which is a persistent inflammation of the middle ear, causing long-term ear discharge (otorrhoea), pain, and tympanic membrane perforation. Its impact is significant, leading to conductive hearing loss, social stigma, educational challenges in children, and, in severe cases, dangerous infections like mastoiditis or meningitis. Prognosis depends on early and proper treatment. Regular ear care, appropriate antibiotics, and surgical repair when needed can control infection and improve hearing. Untreated cases may lead to persistent hearing loss, speech difficulties, or rare but serious complications.
