Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts. That’s why the chronology can sometimes be in disorder.

Sepsis is a life-threatening condition that occurs when an infection spreads into the bloodstream and triggers a widespread inflammatory response in the body. This can lead to organ dysfunction, poor circulation, and, if not treated urgently, septic shock and death. In infants, it may present with fever or low temperature, poor feeding, lethargy, irritability, and convulsions. Severe dehydration due to gastroenteritis occurs when a child loses large amounts of fluids and electrolytes through persistent diarrhoea and vomiting. This leads to reduced circulating blood volume and impaired organ function. Signs include sunken eyes, dry mouth, poor skin turgor, reduced urine output, lethargy, inability to feed, and in severe cases, shock. Oral thrush is a fungal infection of the mouth caused by Candida species. It presents as white patches on the tongue, inner cheeks, or palate, and may cause discomfort, difficulty feeding, and irritability, especially in infants.

Background

Liam is a 3-month-old baby boy and a twin to his brother Lian. They live with their parents in a remote rural area with very limited access to basic services. Their mother is a housewife who stays at home to care for the children, while their father works as a boda boda rider, earning a small and unstable daily income. Unfortunately, his earnings are not sufficient to adequately meet the family’s basic needs, including food, shelter, and healthcare. Recently, both Liam and his twin brother fell seriously ill and required urgent medical attention. However, due to the family’s difficult financial situation, their parents were unable to afford hospital bills, medications, and necessary investigations. This placed the family under significant emotional and financial strain as they struggled to access proper care for their young children. The family learned about Helpster Charity during a local chief’s baraza, where a Helpster volunteer sensitized community members about the program and how it supports underprivileged patients in accessing treatment. Liam was brought to the health facility by his mother, who reported that he had developed diarrhoea characterized by loose, blood-stained stools, with two episodes occurring within the last 12 hours. He had also experienced two episodes of vomiting, high fever, and convulsions. His condition had rapidly worsened—he became weak, lethargic, and unable to breastfeed effectively. He appeared severely ill, with signs of dehydration such as sunken eyes, dry mouth, and reduced activity. He was irritable at times and at other moments unusually quiet, raising concern for worsening illness. His body was hot to touch, and the repeated vomiting and diarrhoea further compromised his condition, making him critically unwell. Following thorough clinical examination and laboratory investigations, Liam was diagnosed with severe dehydration, oral thrush, sepsis, gastroenteritis, and diarrhoea. Due to the severity of his condition, he was admitted for urgent treatment, close monitoring, and further investigations to stabilize his health and prevent complications.

Medical history

Mother brought the child with complains;diarrhea loose stool with blood stained reports of 2 episodes for the last 12hours, vomiting 2episodes, hotness of the body and convulsion. Based on clinical examination and laboratory results, he diagnosed with severe dehydration, oraltrash, sepsis,gastroenteritis and diarrhea. Was admitted for treatment, management and further investigation, after awhile he responded well to medication and discharged when stabilized and in good condition.

Prognosis

The combination of persistent diarrhoea, vomiting, fever, and convulsions indicates a severe systemic infection with significant fluid and electrolyte loss. Sepsis further suggests that the infection has spread into the bloodstream, placing Liam at high risk of complications such as organ dysfunction. His young age makes him particularly vulnerable, and the inability to feed further worsens his condition. He has been admitted for intensive management, including intravenous fluid resuscitation, broad-spectrum antibiotics, antifungal treatment for oral thrush, and supportive care such as temperature control and monitoring for further convulsions. Close observation is ongoing to assess his response to treatment and detect any complications early. With timely and appropriate intervention, Liam is expected to gradually stabilize as hydration is restored and the infection is controlled
