Severe pelvic inflammatory disease (PID) is a serious infection of the female reproductive organs, often involving the uterus, fallopian tubes, and ovaries. It presents with lower abdominal pain, fever, foul-smelling vaginal discharge, nausea, and general malaise. Ultrasound helps identify fluid collections or tubo-ovarian involvement. Early treatment with intravenous broad-spectrum antibiotics, supportive care, and close monitoring is essential to prevent complications such as abscess formation, or sepsis.

Background
Zainab Yakubu is a 17-year-old girl from a low-resource family that often depends on external support for medical care during serious illnesses. She enjoys drawing, helps her friends with schoolwork, and aspires to become a midwife to support women in her community. She presented with a five-day history of lower abdominal pain, fever, and foul-smelling vaginal discharge, associated with nausea, vomiting, and general weakness. On examination, she appeared ill and febrile, with lower abdominal tenderness and cervical motion tenderness on pelvic assessment. She was diagnosed with severe pelvic inflammatory disease and commenced on intravenous broad-spectrum antibiotics, alongside supportive care and pain management.
Medical history
After falling seriously ill with Persistent lower abdominal pain,fever and foul smelling vaginal discharge since 5 days, Zainab struggled to keep up with school and daily life. Her family, worried and without enough money for treatment, turned to Helpster Charity for support. Thanks to Helpster Charity, she was evaluated and investigated in UMD hospital and diagnosed with Severe Pelvic inflammatory disease. She was admitted and had antibiotics,intravenous fluids with other supportive treatment in the hospital for 3 days. Slowly, she recovered, symptoms subsided, and she regained her strength. She was discharged home with her parents, and she can enjoy his childhood and return to school. Prognosis
Severe PID is a serious upper genital tract infection requiring urgent treatment. The patient’s symptoms and examination findings are consistent with advanced infection, and prompt initiation of intravenous broad-spectrum antibiotics is appropriate. Close monitoring is necessary to ensure clinical improvement and prevent complications. With early and adequate treatment, short-term recovery is usually good. However, severe PID may lead to long-term complications such as chronic pelvic pain, infertility, or ectopic pregnancy if not properly managed. Timely intervention significantly improves outcomes.
