Why is treatment sometimes done earlier than the payment?

When a case is urgent and vital, hospitals often proceed with delivering medical care right away to save a life, even before the finalisation of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts. That’s why the chronology can sometimes be in disorder.

About emergency care

Emergency Caesarean surgery secondary to fetal distress secondary to meconium grade II means: An urgent C-section was performed because the baby was showing signs of fetal distress, and this distress was linked to the presence of meconium (baby’s first stool) in the amniotic fluid — classified as grade II (moderate amount)

Background

Event Auma is a young teenage mother of one and a recent secondary school graduate. She lives with her spouse and child in a single-room, mud-walled house. Currently unemployed, she depends on her spouse, who earns a living through casual jobs, to support the family. In her free time, she enjoys listening to the radio while practicing hair plaiting, and she hopes to one day own a salon. During her pregnancy, Event recalls feeling unwell for some time before visiting a health facility, where she was informed that she was expectant. Despite financial constraints, she managed to attend four antenatal clinic visits, often walking for about an hour to the facility due to lack of transport fare. One day, she began experiencing intermittent abdominal pain that persisted throughout the day. By evening, the pain became constant. The following morning, she sought care at a nearby hospital, where she stayed for two days before being informed that she required a Caesarean section. She was then referred to Siaya County Referral Hospital for further management. Upon arrival and assessment at the referral hospital, doctors identified signs of fetal distress as seen by grade II meconium-stained amniotic fluid, a condition that poses significant risk to the baby if not managed promptly. To ensure the safety of both mother and child, the medical team proceeded with an emergency Caesarean section.

Medical history

Event was taken for an emergency caesarean surgery secondary to fetal distress. The surgery was successful. In the post operation ward, she was administered plasil, tramadol , ceftriaxone among other drugs. Her catheter was removed, and ambulation was encouraged after her dressing was removed after 72 hrs. Her wound showed to be healing wellm She was eventually discharged on orals and a clinic date booked for her for review

Prognosis

Event Auma presented with prolonged abdominal pain and fetal distress, confirmed by the presence of grade II meconium-stained amniotic fluid. This indicated that the baby was under stress in the womb and at risk of complications such as meconium aspiration and birth asphyxia. The decision to perform an emergency Caesarean section was timely and appropriate, as it helped prevent potentially life-threatening outcomes for both mother and baby. Event’s prognosis is good. With proper post-operative care, she is expected to recover from the Caesarean section within 4–6 weeks The wound is likely to heal well if infection is prevented and hygiene is maintained
