Peptic ulcer disease is a condition where open sores develop in the lining of the stomach or the upper part of the small intestine due to damage from stomach acid. When the disease is exacerbated, it means the ulcers have become more inflamed, painful, or complicated. Severe dehydration is a life-threatening condition that occurs when the body loses a large amount of fluids and essential salts (electrolytes) and is unable to replace them adequately. It commonly results from persistent vomiting, diarrhoea, high fever, excessive sweating

Background

Victor is a 16-year-old boy who enjoys watching football and dreams of becoming a doctor in the future. He is an orphan living in a children’s home that relies entirely on support from well-wishers for its daily needs and operations. Victor was abandoned by the roadside as an infant and was rescued by a good Samaritan, who reported the incident to the police. He was later placed in the children’s home, where he has grown up. Attempts to trace his biological parents or relatives have been unsuccessful. Recently, Victor began experiencing persistent epigastric (upper abdominal) pain described as a burning sensation, particularly after meals and during the night. This was accompanied by nausea and later progressed to vomiting blood (hematemesis), a sign of a serious underlying condition. A volunteer nurse who occasionally works at the children’s home assessed him and suspected a severe gastric ulcer. She advised the home’s administration to seek urgent medical care. With the help of the volunteer, Victor was taken to the hospital for further evaluation and treatment. After thorough examination and investigations, he was diagnosed with gastritis on a background of a previous history of peptic ulcer disease.

Medical history

Victor started experiencing persistent epigastric (upper abdominal) pain characterized by a burning sensation, especially after meals and at night. This was accompanied by nausea and eventually progressed to vomiting blood (hematemesis), indicating a serious underlying condition. Medical evaluation from a volunteer nurse working part time at the children's home, suggested a severe gastric ulcer causing irritation. The nurse advised the administration to seek medication . With the help from the volunteer he was brought to the hospital for treatment. Thorough Examination and test done, he was diagnosed with Gastritis with a personal history of peptic ulcers. He was put on i.v Esomeprazole 40mg , i.v Hyocine 20mg,i.v DNS 500MG I.V R.L 500MLS , Under observation for 10hrs, then discharged when stable on Esclam kit bd x 14/7 and Gaviscon Susp tds x 5/7. Victor is responding well to treatment, that is acording to Comunication through the Matron on 4th /03/2026 .

Prognosis

Victor’s condition is serious but manageable. The diagnosis of exacerbated peptic ulcer disease indicates that his previously existing ulcers have worsened, leading to significant irritation of the stomach lining and complications such as hematemesis (vomiting blood). This suggests active bleeding from the ulcer, which required urgent medical attention. Victor’s prognosis is good with appropriate treatment and follow-up. Most cases of peptic ulcer disease respond well to medical therapy, especially when the underlying cause is identified and treated. If he adheres to the prescribed medications, maintains a proper diet, and attends follow-up care, the ulcers are expected to heal, and he should recover fully without long-term complications.
