Background
Wendy is the firstborn in her family and a secondary school student. She currently lives with her grandmother and two siblings in a two-roomed semi-permanent house. Her grandmother relies on peasant farming to provide for the family. Despite these challenges, Wendy is determined and hopes to become a nurse after completing her education. Several months ago, Wendy realized that she had missed her menstrual periods. Confused and worried, she confided in her class teacher, who counseled and encouraged her not to lose hope but to remain focused on her education. With this support, Wendy continued attending school and stayed committed to her studies despite the challenges of her pregnancy. As the pregnancy progressed, Wendy remained determined to complete her education and build a better future. By seven months, she was still actively attending school. One evening, while returning home on a motorbike after visiting her sister, Wendy was involved in a serious road traffic accident. A speeding vehicle attempting to overtake lost control and knocked them down. She sustained injuries and was rushed to a nearby hospital by well-wishers. Upon medical assessment, it was discovered that the trauma from the accident had resulted in injuries on her face, neck and abdomen and also an in an intrauterine fetal death. Due to the severity of her condition, since she was also having per vaginal bleeding, doctors performed an emergency caesarean section to manage the situation and save her life.
Medical history
Wendy was brought to the hospital bleeding, having soft tissue injuries and disoriented. She successfully underwent an emergency caesarean surgery having unfortunately lost her child from the impact of the accident. She was taken to the post operation ward for further management, her vitals were monitored quarterly and the hourly. Early ambulation was encouraged, oral sips was initiated after 6hours. Her dressing was removed after 72 hrs, and catheter removed 8hrs . While in the ward dhewa administered IV ceftriaxone, oxytocin, flagyl among other drugs. She responded well to medication and was eventually discharged on orals. A follow up clinic was booked for her which confirmed she was recuperated and had gone back to school.
Prognosis
The prognosis for a 19-year-old undergoing an emergency caesarean section (ECS) for intrauterine fetal demise (IUFD) following a road traffic accident (RTA) is guarded but generally survival-oriented for the mother, provided immediate surgical and trauma care are available. Wendy will receive urgent and prompt medical intervention at Siaya County Referral Hospital, and is expected to make a full recovery with some therapy.
