Why is treatment sometimes done earlier than the payment?

The child has anorectal malformation otherwise known as Imperforated anus, Anorectal Malformations (ARMs) are a group of congenital anomalies in which the anus and rectum do not develop normally. As a result child was not able to pass stool and having abdominal distention. The child also has a colostomy which is a temporary diversion of feaces before correction of the anorectum, which will require reversal.

Background

Habibu is 8months old, he lives with his parents on a room and palour house in Kaduna. The child is the 6th child of the family. His parents are poor. The father is a farmer while his mother is a house wife. The boy was delivered at a government facility via SVD, child was discharged hours following delivery, however at home mother noticed child abdomen was distending and no passage of stool, they presented back to the facility where they were subsequently referred to BDTH where emergency colostomy was done to relieve distention, following surgery child was passing feaces through the stoma. They have been on regular follow up there and were planned for correctional surgery however caregivers could not afford the surgery and were fortunate to be informed about Helpster charity foundation which prompt their presentation to this facility.

Medical history

Habibu Jubril, a 9-month-old boy, was admitted to our facility on 22/7/2026 with a diagnosis of anorectal malformation without fistula and a sigmoid colostomy. He underwent a successful first-stage surgery, Posterior Sagittal Anorectoplasty, under general anesthesia on 23/7/2026, and was well supported with IV antibiotics, analgesics, fluids, and preoperative investigations. During his admission he made significant improvements, and was discharged on 2/8/2026 to continue with oral medications at home. He is now on follow-up for anal dilatation and is being prepared for colostomy reversal once the PSARP site has healed adequately.

Prognosis

Imperforate anus (anal atresia) is a congenital condition where a baby is born with a missing, blocked, or abnormally positioned anal opening. It is a form of anorectal malformation that ranges from mild (a thin tissue membrane blocking the opening) to complex cases involving abnormal connections (fistulas) to the urinary or reproductive tracts. Has a good prognosis because of the earlier colostomy done at the second day of life, however with colostomy reversal there’s risk of complications such as anastamotic breakdown or leakages, other complications that may arise could be wound breakdown, and anal stenosis.
