About infections
Severe malaria and lower respiratory tract infection (LRTI) is a serious coexisting condition associated with high morbidity, especially in children. Severe malaria, commonly due to Plasmodium falciparum, presents with high fever, anemia, convulsions, and possible organ dysfunction. LRTI, including pneumonia or bronchopneumonia, presents with cough, fast breathing, chest indrawing, and respiratory distress. Patients may appear very ill, febrile, weak, and hypoxic. The combination worsens oxygen delivery due to anemia and lung involvement. Diagnosis is confirmed by blood tests and clinical examination.

Background
Sa'adatu is a 2 year old female child, she is the 4th child among 5 siblings, she is living with her parents in a community outside Bauchi. The father is not currently working, the mother is a house wife. They struggle to feed everyday. They live in a 2 rooms apartment having a pit toilet. They use water from the borehole for their daily activities. They don't have access to electricity. The children could not continue going to school because there is no school fees. Presenting complain: High grade fever, nasal blockage, low appetite, loose mucoid stooling. Sa'adatu has been having fever for the past 5 days which was high grade, the mother noticed her stooling to be loosed accompanied with mucoid, she eventually started having nasal blockage and very low appetite. On examination, a 2-year-old female child, very ill looking, pale, febrile with temperature 39.3 degree Celsius, anicteric, acyanosed. She was diagnosed with Severe Malaria. Due to poverty, the family were not able to take her to the hospital, they reached out to Helpster Charity through a volunteer. The child was admitted and evaluated thoroughly for urgent medical treatment.
Medical history
Sa'adatu has been having fever for the past 5 days which was high grade, the mother noticed her stooling to be loosed accompanied with mucoid, she eventually started having nasal blockage and very low appetite. On examination, a 2-year-old female child, very ill looking, pale, febrile with temperature 39.3 degree Celsius, unicteric, acyanosed. She was diagnosed with severe malaria. She was admitted and was managed with antimalarial injection, injection antibiotics and other supportive until patient was stable. Fever stopped, feeding continued well and was discharged with some oral medications. Patient is to return for follow up after 1 week.
Prognosis
Sa'adatu has been having fever for the past 5 days which was high grade, the mother noticed her stooling to be loosed accompanied with mucoid, she eventually started having nasal blockage and very low appetite. On examination, a 2-year-old female child, very ill looking, pale, febrile with temperature 39.3 degree Celsius, unicteric, acyanosed. She was diagnosed with severe malaria. Management includes prompt antimalarial therapy, antibiotics, oxygen support, fluids, and close monitoring to prevent complications and reduce mortality. Prognosis is good with early and appropriate treatment.
