Schistosomiasis is caused by parasitic worms (Schistosoma species), commonly transmitted through contact with contaminated freshwater. In urinary schistosomiasis, the parasites primarily affect the bladder and urinary tract. Chronic cystitis is a long-term inflammation of the bladder that causes persistent pelvic pain, frequent urges to urinate, and ongoing discomfort during urination. Malaria is a serious disease caused by parasites spread to people through the bites of infected female Anopheles mosquitoes. It typically presents with high fever, chills, and flu-like illness, but can quickly become life-threatening if not treated with effective antimalarial medication

Background

Jibrin A seven-year-old boy, the second child in a family of two children, lives with his parents in Amshi village. The family resides in a small single-bedroom house, reflecting their modest living conditions. His father is a subsistence farmer, while his mother is a petty trader. Together, they earn a combined income of about 9,000 naira, which they rely on for their daily survival. The older sister is 30 years and acts as the guardian because she understands more than her parents. He attends school and nurtures a strong ambition to become a soldier when he grows up, aspiring to serve and protect his country. Jibrin presented to Ayaji hospital with several days of painful urination (dysuria), blood in urine (hematuria), and dizziness. He had previously received treatment from patent medicine stores, including antiprotozoal drugs and antibiotics, without improvement. On examination, he was conscious but pale, with low blood pressure, while other systems were unremarkable. Laboratory findings revealed malaria parasites. Diagnosis: Schistosomiasis complicated by chronic cystitis, with coexisting malaria infection.

Medical history

Jibrin a 10-year-old child diagnosed with schistosomiasis was managed on an outpatient basis after clinical evaluation and laboratory confirmation. He was commenced on praziquantel as the definitive treatment for the parasitic infection. Supportive care was also provided, including analgesics for abdominal discomfort and adequate hydration. Health education was given to the caregiver on prevention measures such as avoiding contact with contaminated freshwater sources and maintaining proper sanitation. The child responded well to treatment with gradual resolution of symptoms, improved appetite, and overall clinical recovery. He was reviewed during follow-up and remained stable with no complications.

Prognosis

Based on the patient’s history, clinical presentation, and laboratory findings, the diagnosis of Schistosomiasis complicated by Chronic cystitis was made. The presence of hematuria, dysuria, and laboratory confirmation strongly supports involvement of the urinary tract by the parasitic infection. eated initially. When the infection is prolonged or untreated, it results in repeated irritation and damage to the bladder lining. This chronic irritation eventually leads to chronic cystitis, which is a long-standing inflammation of the bladder.
