Praise D., 21

Praise is a 21 year old young adult currently staying with his parents in a community around Bauchi state. He has finished his secondary education awaiting to further his education but no money at the moment. The father is not currently working, the mother is a house wife. They live in a 2 rooms apartment having a pit toilet, well water and not much income. David was well until 7 days prior to presentation that he started having high grade fever that is continues throughout the day. At the same time, he noticed swelling of his lower limb that started around a boil in his thigh. Swelling rapidly increased to involve the entire limb with blistering of skin and darkening of skin around the initial site of boil. Condition continued to get worse despite home remedies necessitating him to seek help in this facility. Examination revealed an acutely ill-looking young man in painful distress, febrile with temperature of 39.8. The right lower limb is grossly edematous with necrotic skin on proximal anteromedial thigh about 20 x 30cm, widespread bullous lesions on the rest of the limb. Diagnosis of Right Necrotizing Fasciitis was made.

David is a 21 years old male who presented with 7 days history of high grade fever that is continues throughout the day. At the same time, he noticed swelling of his lower limb that started around a boil in his right groin. Swelling rapidly increased to involve the entire limb with blistering of skin and darkening of skin around the initial site of boil. Condition continued to get worse despite home remedies necessitating him to seek help in this facility. Examination revealed an acutely ill-looking young man in painful distress, febrile with temperature of 39.8 degrees. The right lower limb was grossly oedematous with necrotic skin on proximal anteromedial thigh about 20 x 30cm, widespread bullous lesions on the rest of the limb. Diagnosis of right lower limb necrotizing fasciitis was made and he worked up and had two surgeries to debride the wound then skin graft was done to cover the defect after three weekds on admission. Main problems encountered during care of the patient was malnutrition with low serum albumin and anaemia which requires repeated corrections. Despite the challenges, patient did well, graft take was excellent, patient work out of the hospital on discharge happy. If not for the intervention of Helpster, this patient could have died of septisemia or loose the affected limb. Necrotizing fasciitis in a 21-year-old is a critical, life-threatening surgical emergency that requires immediate hospitalization, high-dose intravenous antibiotics, and urgent surgical removal of infected tissue. Despite the patient's young age, this aggressive "flesh-eating" infection can spread within hours, completely overwhelming a healthy immune system. If treatment is delayed, severe complications rapidly develop, including septic shock, multiple organ failure, permanent scarring, limb amputation, and death. Prognosis for Praise's condition depends on early diagnosis and prompt treatment. With aggressive surgical debridement, antibiotics, and supportive care, recovery is possible.
