Why is treatment sometimes done earlier than the payment?

When a case is urgent and vital, hospitals often proceed with delivering medical care right away to save a life, even before the finalisation of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts. That’s why the chronology can sometimes be in disorder.

About urology

Aziba Hossain has been diagnosed with right-sided gross hydronephrosis with acute pyelonephritis. This means her right kidney has become severely swollen due to blockage or urine buildup, while a serious kidney infection has developed at the same time. She has been suffering from high fever, vomiting, weakness, irritability, poor feeding, and reduced urine output, which indicate that the infection is affecting her overall condition. Without timely treatment, the infection and kidney swelling may worsen, leading to permanent kidney damage, severe sepsis, or other life-threatening complications.

Background

Aziba Hossain, a 1-year-4-month-old baby girl, lives with her parents, elder brother, and elderly grandfather in a poor rural family where her father earns only about 9,000 BDT per month by selling bus tickets temporarily, making it extremely difficult to afford expensive medical treatment. She is a gentle and innocent child whose laughter brings happiness to her family, but her sudden severe illness has left everyone terrified and helpless. Aziba developed high fever, vomiting, weakness, poor feeding, and reduced urination, and doctors later diagnosed her with severe swelling of the kidney (gross hydronephrosis) along with acute pyelonephritis, a dangerous kidney infection. Her mother and grandfather are constantly worried as the family has no savings and struggles even to manage daily meals and medicines. They learned about Helpster Charity through volunteer ABM Alamgir, as without urgent treatment, the infection may spread, causing kidney failure, severe sepsis, permanent kidney damage, and life-threatening complications.

Medical history

Aziba Hossain was admitted to the hospital on 06-04-2026 with high fever, repeated vomiting, dehydration, abdominal swelling, and severe weakness caused by a serious kidney-related condition. After admission, the doctors conducted several important investigations including blood tests, urine examination, ultrasonography, CT urogram, and renal function assessments to identify the underlying problem and evaluate the condition of her right kidney. She received intravenous fluids, antibiotics, pain management, and supportive treatment under close medical observation. As her condition required surgical management, an operation was successfully performed on 02-05-2026 to treat the underlying renal complication and improve urinary drainage and kidney function. Following the surgery, Aziba gradually responded well to treatment, her fever subsided, vomiting stopped, and her overall physical condition improved steadily. After satisfactory postoperative recovery and stabilization, she was discharged from the hospital on 10-05-2026 with advice for regular follow-up, medication, proper nutrition, and continued monitoring for healthy recovery.

Prognosis

Doctors have advised Anderson-Hynes (AH) Pyeloplasty to correct the blockage affecting Aziba’s right kidney and restore normal urine flow. With timely surgery, proper antibiotics, and follow-up care, the infection can be controlled, kidney swelling can gradually reduce, and her kidney function may improve significantly. Successful treatment will help relieve pain, improve feeding and growth, and give her a chance for a healthy childhood. However, if left untreated, the blockage and infection may continue to damage the kidney, leading to permanent kidney failure, repeated severe infections, sepsis, and worsening physical weakness. In severe cases, these complications can become life-threatening and may seriously affect her future growth and survival.
