Severe pneumonia is a serious lung infection that causes significant breathing difficulty due to inflammation and fluid in the lungs. It presents with fast breathing, chest indrawing, high fever, cough, and inability to feed or speak well. Severe asthma attack (acute exacerbation) is a life-threatening condition where the airways become severely narrowed due to inflammation and muscle constriction, leading to wheezing, shortness of breath, chest tightness, and use of accessory muscles. Both conditions impair oxygen delivery and require urgent medical treatment.

Background
Sebastian Omondi is the fourth-born child in his family and has not yet started school. He is a partial orphan, having lost his father three years ago after a tragic attack when he was attacked by thugs. He lives with his mother, who struggles to meet basic needs such as food, clothing, healthcare, and shelter. She depends on occasional menial jobs, and at times the family survives on only one meal a day or goes without food. They live in a single mud-walled room with poor living conditions. Despite these hardships, Sebastian is a playful, curious, and friendly child who enjoys playing with other children using improvised toys. Sebastian fell ill after waking up weak, with chest pain, fever, chills, and general body weakness. He developed a persistent cough, wheezing, difficulty breathing, and was unable to speak well. His condition worsened with nasal flaring and use of accessory muscles. With no money for transport, his mother, with help from a neighbor, walked him to the hospital. He was received in severe respiratory distress with high fever and urgently admitted for lifesaving treatment.
Medical history
Sebastian came to the hospital with complaints of coughs, chest pain and difficulty in breathing. Tests and examination done revealed he was suffering from severe pneumonia and also had a severe asthmatic attack thiswarranted admission to the ward for further management. He was administered IV xpen, gentamicin, paracetamol among other drugs. He responded well to medication and was eventually discharged on orals.
Prognosis
Sebastian presented with features of severe pneumonia with possible acute severe asthma, including high fever, respiratory distress, wheezing, and difficulty breathing. He was promptly admitted and started on emergency management, including oxygen therapy, antibiotics, bronchodilators, and supportive care. At assessment, he is stable but requires close monitoring due to the severity of his condition. The prognosis is fair to good. However, without prompt care, the condition could lead to respiratory failure, making early intervention critical for recovery.
