Brighton S., 7

Brighton is the third-born child in a family of four children. He lives with his parents and siblings in a modest two-roomed, mud-walled house. Both of his parents are peasant farmers with limited and unstable income. When in good health, Brighton is an active and cheerful child who enjoys playing football with his friends and climbing trees. He dreams of becoming a teacher in the future. According to his parents, the incident occurred on a day they had gone to work on a neighbour's farm. They later received distressing news that Brighton had been playing with other children when he climbed a tree. As he was descending, he slipped and fell onto a sharp, protruding tree stump, which pierced his private area. The injury caused a severe tear to the scrotum, leaving the testes exposed. A nearby neighbor who witnessed the incident quickly rushed Brighton to a nearby health facility and informed his parents. Upon arrival, Brighton’s parents were advised that his condition required urgent specialized care, and he was immediately referred to Siaya County Referral Hospital for emergency surgery. The family had earlier learnt about Helpster Charity during a chief’s baraza held in their area.

Brighton was brought to the hospital with scrotal bleeding secondary to a fall from a height. He fell on a shark stick sustaining ruptured scrotum. He was taken for an emergency surgery which was a success. He was taken to the ward where he was administered IV ceftriaxone, morphine, pcm among other drugs. He responded well to medication and was eventually discharged on orals and a clinic set for after every three days for dressing of the wound.

Brighton sustained a severe penetrating scrotal injury with exposure of the testes, placing him at high risk of infection, tissue damage, and life-threatening complications. The open wound significantly increases the likelihood of bacterial contamination, which can rapidly progress to severe infection, including sepsis if not urgently managed. There is also a risk of tissue necrosis due to compromised blood supply, which, if it occurs, can spread and become fatal. He requires immediate surgical intervention for wound repair, debridement, and infection control. With prompt and appropriate treatment, the prognosis is good; however, any delay in care greatly increases the risk of severe complications and poor outcomes.
