About emergency care

An emergency cesarean section for non-reassuring fetal status (NRFS)—often termed fetal distress—is a rapid intervention to prevent fetal hypoxia, acidosis, or death. It is prompted by abnormal fetal heart rate patterns (e.g., severe bradycardia) suggesting inadequate oxygen supply. Immediate delivery is critical, with the fastest method determined by the team. Preterm drainage of liquor (preterm premature rupture of membranes) is the leakage of amniotic fluid from the uterus before 37 weeks of pregnancy, before labour begins. It increases the risk of infection, preterm birth, and complications for both mother and baby.

Background

Brenda lives with her husband and child in a one-room mud-walled house. She is a housewife and depends on her husband, whose work as a fisherman is irregular and often provides only hand-to-mouth income, making it hard to meet basic needs. Brenda left school in Form Three due to financial challenges and got married at a young age, though she still hopes to one day work in a hospital. About a month after missing her period, Brenda developed nausea and fatigue and sought care at a local health centre, where her pregnancy was confirmed. She attended three antenatal visits, walking over an hour each time. During one visit, she learned about Helpster Charity from a poster and a nurse’s explanation. She remained hopeful for a normal delivery. Later, she became suddenly weak with drainage of watery fluid. On evaluation, she had leaking liquor and fetal distress. She was urgently referred to Siaya County Referral Hospital, where she underwent an emergency caesarean section to save both her life and that of her baby.

Medical history

Brenda was taken for an emergency caesarean surgery, the surgery was a success and she was taken to the post -op ward. While in the ward she was administered ceftriaxone,flagyl, morphine, among other drugs. Her catheter was removed after 2 days, and ambulation encouraged. The dressing on her surgical site was removed after 72 hrs. She was able to respond well to medication, and she was eventually discharged on orals. A follow up clinic was booked for her to ensure she was healing well.

Prognosis

Brenda presents with preterm drainage of liquor and signs of fetal distress, indicating a high-risk obstetric emergency. She is currently under close monitoring, with efforts focused on stabilizing her condition and preparing for urgent intervention. The fetal distress suggests that the baby may not be receiving adequate oxygen, while the leakage of amniotic fluid increases the risk of infection and preterm complications. At this stage, the prognosis is guarded, as both maternal and fetal outcomes depend on timely intervention. With prompt and appropriate management, there is a good chance of improving outcomes and ensuring the safety of both Brenda and her unborn child.
