Isaac is a 1yo old male child, living with his parents and 3 siblings. Father is a labourer and mother a house wife. They live in abject poverty struggling everyday to get what to eat. They have 2 rooms, a pit toilet, a well and no access to electricity. The monthly income in the family is 20,000. Isaac is an 11 months old male who presented with history of persistent cough, high grade fever, and difficulty breathing of two weeks that has not responded to home remedies. He lives in a small room with poor ventilation with his parents and other siblings. No history of person with chronic cough though. No significant weight loss or drenching night sweat. Examination shows an acutely ill looking child with difficulty breathing, SPO2 of 90 in room air, respiratory rate of 64cpm temperature of 39.8oC. Widespread crepitations noted in mid and lower lung fields bilaterally. Diagnosis of Severe Bronchopneumonia was made.

Following swift response to treatment, he did well and become stable after management on some injection antibiotics and other supportive care was given. The family remains grateful for saving the life of their child. They left so happy.

Isaac presented with fever, cough and signs of difficulty breathing and was diagnosed with Severe Bronchopneumonia. The case is admitted for immediate treatment to prevent complications. Severe bronchopneumonia can lead to life-threatening respiratory failure and Acute Respiratory Distress Syndrome (ARDS). Additionally, the infection can spread into the bloodstream or surrounding chest cavity, causing septic shock, lung abscesses, or empyema (pus accumulation around the lungs). The prognosis for severe bronchopneumonia is excellent if targeted antibiotics and supportive therapies, such as oxygen and hydration, are started early. However, delays in treatment significantly increase the risk of serious complications, leading to a higher chance of prolonged illness or mortality.
