Post-typhoid hepatitis with conjugated hyperbilirubinemia is a rare pediatric complication where Salmonella Typhi infection triggers liver inflammation and bile duct dysfunction, leading to a buildup of direct bilirubin and noticeable jaundice. It presents alongside typical typhoid symptoms like prolonged fever, hepatomegaly, and dark urine, indicating acute hepatic injury rather than direct hemolysis. While the liver damage is usually reversible with proper antibiotic therapy, potential complications include acute liver failure, hepatic encephalopathy, gallbladder perforation, and a prolonged carrier state.

Background
Saminu Yakubu is a five year old boy from a Bayan Kasuwa settlement where clean water and healthcare are scarce. He is the second of three children born to subsistence farmers, and he is known as a cheerful and energetic child who loves playing with his older brother and helping his mother collect firewood. About two weeks before presenting to Adari, Saminu developed a high fever with headache and abdominal pain, for which he was treated at home with paracetamol and herbal remedies. Although the fever lasted about ten days before resolving, he later developed new concerning symptoms including jaundice (yellowing of the eyes), dark urine, pale stools, marked fatigue, and poor appetite, indicating a likely underlying liver involvement requiring urgent medical evaluation. He was evaluated and diagnosed with post-Typhoid Hepatitis complicated with Conjugated Hyperbilirubinemia.
Medical history
Saminu has not received proper treatment due to financial constraints and remains ill with jaundice, dark urine, fatigue, and poor appetite, with his mother unable to afford essential tests or care. She is seeking help from Helpster Charity Foundation. With support, he can undergo diagnostic tests to confirm hepatitis A and receive supportive treatment including IV fluids, nutritional support, and liver care. With proper management, he is expected to recover fully over time, with resolution of jaundice, return of appetite, and full restoration of health.
Prognosis
Dr. Zainab M. Ngubdo noticed that Saminu presents with a biphasic illness of 10 days of fever, headache, and abdominal pain followed by jaundice, dark urine, pale stools, severe fatigue, and poor appetite, highly suggestive of acute viral hepatitis, likely hepatitis A. He requires urgent evaluation with liver function tests and hepatitis serology for confirmation, along with supportive care including hydration, nutritional support, and close monitoring for signs of acute liver failure. Prognosis is generally good, with most children recovering fully within 4–8 weeks. However, without proper supportive care, he risks dehydration and progression to acute liver failure, which carries a significantly worse prognosis.
