Reduced fetal movements in a patient with premature rupture of membranes (PROM) refers to a decrease in the baby’s usual activity felt by the mother after the membranes have ruptured before delivery. PROM occurs when the amniotic sac breaks before labour is fully established, causing leakage of fluid. Reduced fetal movements in this setting may indicate fetal distress, reduced amniotic fluid, infection, or compromised oxygen supply to the baby, and requires urgent medical evaluation and close monitoring to prevent serious complications for both mother and child.

Background

Dorothy is a 16-year-old girl from a humble rural background and currently a Grade 9 student at a local public junior secondary school. Despite financial hardships, she remains disciplined, respectful, and hardworking both at school and at home. She walks long distances daily to attend classes and studies with limited learning materials, yet continues striving for a better future through education. Alongside her studies, she assists with household chores before and after school. Her family survives on small and irregular sources of income, making access to proper medical care difficult. Dorothy presented to the hospital in labour accompanied by her mother at 39 weeks of gestation as a primigravida. She complained of reduced fetal movements, drainage of fluid, and mild abdominal pain that had not increased in frequency or intensity. Findings revealed fetal malpresentation in breech position, and there was concern because fetal movements were significantly reduced prior to arrival at the facility. Due to the risks of possible fetal compromise, the medical team recommended an emergency Caesarean section.

Medical history

Dorothy came to the facility in labour accompanied by her mother presenting with primigravida at 39 weeks , not perceiving fetal movement normally as before, drainage of liquid, mild abdominal pain not increasing in frequency and intensity. On examination and laboratory investigation, she was diagnosed with reduced fetal movement and breech presentation. Due to this complications medical doctors made decision for emergency ceaserian section to be contacted immediately for the purpose of saving lives of mother and the baby. Surgical procedure was done successfully and after awhile she regained and discharged when stable and in good condition.

Prognosis

Dorothy presented with reduced fetal movements and premature rupture of membranes, findings that raised concern for possible fetal compromise. Assessment confirmed breech presentation, increasing the risk of complications during delivery. Due to the reduced fetal activity and leakage of amniotic fluid, emergency Caesarean section was recommended to improve maternal and fetal outcomes. Prompt surgical intervention helped reduce the risk of prolonged fetal distress, infection, and delivery-related complications. Both mother and baby require close post-operative monitoring, but the timely management provided a reassuring chance for recovery and stabilization.
