Severe acute malnutrition (SAM) is a life-threatening condition caused by extreme nutrient deficiency. It is defined by severe wasting (low weight-for-height), a very low mid-upper arm circumference (MUAC), or bilateral pitting edema. It compromises immunity, making children highly susceptible to fatal infections. A Grade 3 rectal prolapse occurs when the full thickness of the rectal wall protrudes through the anus during physical exertion (like straining or walking) and requires manual pushing to return to its normal position.

Background
Brighton is the youngest child in his family and lives with his teenage single mother, elder sibling, uncle, and grandmother in a small two-roomed mud-walled house. His mother is a high school graduate who could not proceed to college due to lack of school fees and unemployment. The family depends mainly on his grandmother, a peasant farmer with very little income, making it difficult to meet basic needs and access healthcare. Brighton had been unwell for some time with persistent cough, abdominal pain, recurrent fever, and later developed difficulty passing stool. His mother initially sought treatment at a nearby health centre where he received medication and was discharged home, but his condition did not improve. As his health worsened, the family became increasingly concerned. His grandmother sought help from the local Community Health Promoter (CHP), who advised that Brighton be taken to Siaya County Referral Hospital for specialized care. However, his mother feared seeking treatment because the family lacked health insurance and could not afford hospital costs. The CHP later informed them about Helpster Charity, which supports vulnerable children with medical care.
Medical history
Brighton was brought to the hospital with complaints of abdominal pains, inability to pass stool .He was diagnosed with severe acute malnutrition and rectal prolapse, which warranted admission. While in the ward the nutritional team monitored him round the clock. He was administered ceftriaxone, paracetamol, treated on full liquid diet, administered ORs at 42mls 3 hourly. He responded well to treatment and was eventually discharged on orals and a follow up clinic booked for her
Prognosis
The prognosis for this 8-month-old infant diagnosed with Severe Acute Malnutrition (SAM) and Grade III rectal prolapse is guarded but hopeful with prompt and appropriate treatment. Severe malnutrition places the child at high risk of infections, delayed growth, and other serious complications, while the rectal prolapse may require medical or surgical management. With nutritional rehabilitation, treatment of underlying conditions, and close monitoring at Siaya County Referral Hospital, the child has a good chance of recovery. Continued follow-up and adequate nutritional support after discharge will be essential for sustained improvement and healthy growth.
