About pregnancy
Pre-Eclampsia Toxemia. It is a serious condition characterized by high blood pressure and signs of damage to another organ system (often the liver or kidneys), usually developing after the 20th week of pregnancy. PROM (Premature Rupture of Membranes) is when the amniotic sac breaks (“water breaks”) before labor begins. If this happens before 37 weeks, it is called PPROM (Preterm PROM). It requires immediate medical attention to reduce the risk of severe infections and premature delivery. An emergency cesarean surgery secondary to PROM in breech presentation is a prompt surgical delivery because the amniotic sac has ruptured before labor begins, and the baby is oriented with its buttocks or feet—rather than its head—facing the birth canal, necessitating immediate extraction to protect both mother and child.

Background
Agnes is the second born in her family and lives with her father and younger sister in a two-room mud-walled house. Both her mother and elder sibling are deceased. She dropped out of school in Grade 9 due to financial difficulties and currently depends on her father, who works as a casual labourer. Despite the challenges, she hopes to one day own a salon business. Agnes initially began feeling unwell, fatigued, and generally unwell, and later missed her menstrual period. A friend assisted her to confirm pregnancy using a test kit, which was positive. When she informed the father of the baby, he declined responsibility and abandoned her. With her father’s support, she attended four antenatal clinic visits. During late pregnancy, she developed abdominal pain and severe headaches and sought care at a nearby facility, where she was found to have elevated blood pressure and was referred to Siaya County Referral Hospital. At the referral hospital, she went into labour and was found to have a breech presentation. Due to the associated risks, she was taken for emergency caesarean section for safe delivery.
Medical history
Agnes was brought to the hospital with complaints of abdominal pains and high blood pressure. She was diagnosed with pre eclampsia and had a baby in breach presentation and was due for an emergency surgery. Her surgery was a success, she was taken for post operation management, she was administered ceftriaxone, flagyl, tramadol among other drugs. Her vitals were monitored hourly and then quarter hourly. Her catheter was removed after 24 hours and given oral sios after 6hours. She was eventually discharged on orals and a follow up clinic booked for her.
Prognosis
The prognosis for a mother who undergoes an emergency caesarean section due to premature rupture of membranes (PROM) with the baby in breech presentation, and severe pre eclampsia is generally good when prompt surgical intervention is performed and there are no major complications. Most mothers recover well with appropriate postoperative care, including pain management, infection prevention, and monitoring for bleeding. Agnes surgery was successful and is set to make a full recovery with adequate proper post operation care
