Nasiru I., 4

Four-year-old Nasiru Ibrahim belongs to a nomadic pastoralist family in Jigawa State, living in temporary grass huts that change location every few months. Due to his family's constant movement and dry-season food shortages, he is small for his age, has never attended school, and has received no routine vaccinations. He was an active and healthy child who loved following his older brother to graze sheep and chasing chickens around their settlement. About two weeks before arriving at Adari Medical Clinic, Nasiru suddenly developed a very high fever that made his body feel hot like fire. He shivered violently and refused to eat anything. His mother gave him herbal teas and local remedies for one week, but his condition only worsened. The fever never went away completely, and then his left thigh began to swell and turn red. Nasiru stopped walking entirely and cried loudly whenever anyone tried to touch his leg or pick him up. He lay on a mat all day looking miserable. His mother became terrified when she noticed he could not even stand. His father borrowed money from neighbors for transport and rushed Nasiru to the clinic on 14th May.

Nasiru arrived at the clinic looking pitiful, unable to move his swollen leg. His mother carried him everywhere. On day one, nurses started intravenous artesunate and antibiotics while he screamed. His mother sang lullabies to calm him. By day three, the swelling reduced and Nasiru stopped crying when his leg was examined. He ate a small bowl of porridge with enthusiasm. On day five, he surprised everyone by standing up holding the bed rail. His mother burst into tears of joy. Nasiru was diagnosed with co-existing severe Plasmodium falciparum malaria and acute Staphylococcus aureus osteomyelitis, which caused confirmed lytic bone damage in his left thigh. Complicated by malnutrition, dehydration, anemia, and thrombocytopenia, his condition required a critical five-day hospital admission for intravenous therapies and stabilization. Nasiru's co-existing infections resolved well during his admission with intravenous artesunate and antibiotics, but he requires four weeks of oral cloxacillin at home to prevent chronic bone infection or joint destruction. His overall prognosis for a complete recovery is favorable, and with consistent care and regular follow-ups to monitor bone healing and malnutrition, he is expected to walk and run normally again.
