Failure to progress in labour due to persistent occipito-posterior position occurs when the baby remains positioned with the back of the head facing the mother's back instead of the front. This abnormal position can make labour longer, more painful, and less effective, preventing the baby from descending normally through the birth canal. As a result, cervical dilation and delivery may not progress adequately, often necessitating assisted delivery or Caesarean section to prevent complications for both the mother and baby.

Background
Conslata is the third-born child in a family of three. Her parents separated several years ago, and she currently lives with her mother, a casual laborer, while her father's whereabouts remain unknown. She is a Form Four student and her education is supported through bursaries and assistance from well-wishers. The family lives in a modest two-room mud-walled house. During her free time, Conslata enjoys listening to gospel music and dreams of becoming a fashion designer in the future. Conslata realized she was pregnant after missing her menstrual periods for two months and experiencing symptoms such as dizziness, fatigue, and social withdrawal. She kept the pregnancy to herself until a concerned neighbor noticed changes in her behavior and informed her mother. Suspecting pregnancy, her mother took her to a nearby health facility under the pretext of seeking medical care. Following evaluation, the pregnancy was confirmed. Although initially distressed by the news, her mother became supportive and ensured that Conslata attended antenatal care clinics regularly. The individual responsible for the pregnancy disappeared after learning about it. On 28/05/2026, Conslata developed severe back pain and abdominal pain. Her mother promptly took her to Siaya County Referral Hospital, where she was admitted in labour. Due to failure of labour to progress, she underwent emergency surgical intervention for safe delivery.
Medical history
Consolata was brought to the hospital with complaints of abdominal pains , she underwent an emergency caesarean surgery secondary to prolonged labour which was successful. She was admitted to the ward for post operation management. Her vitals were Mon 1/4 hourly till she was fully stable. She later started her oral sips after 6 hrs, with the catheter being removed all together . Ambulation was encouraged as she was also given ceftriaxone, metronidazole, paracetamol among other drugs which she retained. She was eventually discharged on orals and a follow up clinic booked for her.
Prognosis
Consolata was admitted in labour but experienced failure of labour to progress due to a persistent occipito-posterior fetal position. This condition increased the risk of prolonged labour and associated maternal and fetal complications, necessitating emergency surgical intervention. The timely Caesarean section helped ensure a safe delivery and prevented further complications. Prognosis is good
