A child with severe malaria is experiencing a serious and potentially life-threatening infection caused by Plasmodium parasites, which are transmitted through the bite of infected mosquitoes. Severe malaria occurs when the infection affects vital organs or causes serious complications such as severe anaemia, difficulty breathing, convulsions, or altered consciousness. The child typically presents with high fever, severe weakness, headache, dizziness, vomiting, loss of appetite, body pains, and general malaise. In cases where severe anaemia develops, the child may appear very pale, tired, have rapid breathing, and experience difficulty carrying out normal activities. Some children may also develop breathing difficulties, dehydration, or convulsions. This condition is particularly dangerous because the parasites multiply rapidly in the blood, destroying red blood cells and reducing the body's ability to transport oxygen. If not treated promptly, severe malaria can lead to severe anaemia, brain involvement (cerebral malaria), organ failure, coma, and death. Immediate medical attention is required and may include intravenous or injectable antimalarial medications, blood transfusion for severe anaemia, intravenous fluids, fever management, nutritional support, and close monitoring. Early diagnosis and treatment are essential to prevent serious complications and improve the child's chances of a full recovery.

Background

Twalha is a 10-year-old boy in Grade 5 and the fourth-born in a family of seven children. He lives with his family in a rural area under the care of a guardian who is a small-scale farmer. The family survives on a modest income from the sale of vegetables and other farm produce, earning approximately KShs 1,000 per month. They live in a simple two-room mud-walled house with earthen floors, have no electricity, and rely on borehole water for their daily needs. Despite these challenges, Twalha is a cheerful, respectful, and hardworking child who enjoys attending school, playing football, and helping with household and farming activities. He is eager to learn and dreams of becoming a teacher so that he can support his family and contribute to his community. Twalha had been in good health until about two weeks before admission when he developed persistent headaches, dizziness, and increasing difficulty carrying out his normal daily activities. His guardian initially sought treatment from a nearby chemist, but his condition failed to improve and gradually worsened. Concerned and unable to afford hospital care, the guardian sought help after being advised by a member of his mosque to visit Al Ihsaan Health Centre, where Helpster Charity supports vulnerable patients. Upon assessment, Twalha was found to require further investigations, close monitoring, and treatment, leading to his admission for medical care.

Medical history

Twalha was admitted to Al Ihsaan Health Centre after being diagnosed with severe malaria. Through the support of Helpster Charity Organization, he received timely and comprehensive medical care at no cost to his family. Upon admission, he was started on appropriate antimalarial medication to eliminate the malaria parasites, along with intravenous fluids to prevent dehydration and medications to reduce fever and relieve pain. He was closely monitored by the medical team throughout his stay. During treatment, Twalha responded well to the medications. His fever gradually subsided, the headaches and dizziness resolved, and his strength and appetite began to return. With continued medical care, adequate nutrition, and observation, his condition steadily improved. After completing the prescribed course of treatment, Twalha recovered well and was discharged in a stable condition. He was able to resume his normal daily activities and continue his education. Prognosis

The doctor’s prognosis for a child with severe malaria complicated by anaemia is generally favorable if the condition is diagnosed early and treated promptly. With appropriate medical care, including antimalarial medications to clear the malaria infection, blood transfusion when necessary to correct severe anaemia, intravenous fluids, nutritional support, and close monitoring, the child's condition is expected to improve significantly. As treatment takes effect, the fever subside will, energy levels gradually return, appetite improve, and Twalha's blood levels recover. Early intervention helps prevent serious complications.
