Severe pneumonia is a serious infection of the lungs that causes inflammation and fluid accumulation in the air sacs, making breathing difficult. It commonly presents with high fever, persistent cough, chest indrawing, rapid breathing, poor feeding, weakness, and reduced oxygen supply to the body. Without prompt treatment, it can result in life-threatening complications. Severe malaria is an advanced form of malaria in which the infection affects major body systems. It may cause very high fever, severe weakness, repeated vomiting, dehydration, anemia, breathing difficulties, confusion, convulsions, or loss of consciousness. It is a medical emergency that requires urgent hospital treatment.

Background
Sheryl is a four-year-old girl who is cheerful, active, and enjoys interacting with people around her. She is friendly, playful, and loves spending time with other children, bringing joy to her family and community. Sheryl lives with her mother in a rural village. Her mother is the sole breadwinner and depends on casual labor jobs for income. The earnings are often low and unpredictable, making it difficult to meet all household needs. Most of the income is spent on food and other basic necessities, leaving little for healthcare and other essential expenses. Sheryl was brought to the health facility after experiencing persistent vomiting, high fever, chills, cough, loss of appetite, and refusal to feed for four days. Her condition progressively worsened, leaving her weak, lethargic, and unable to carry out her normal playful activities. The continued vomiting and poor feeding raised concerns about dehydration and worsening illness, prompting her caregiver to seek urgent medical attention. Following clinical examination and laboratory investigations, Sheryl was diagnosed with severe malaria and pneumonia. Due to the seriousness of her condition, she was admitted for treatment, close monitoring, management, and further investigations to support her recovery.
Medical history
Sheryl was brought to the facility unwell with complains of; difficult in breathing, cough, fever, vomiting everything, Chillis and loss of appetite for the last four days. She went through clinical examination and laboratory investigation and based on findings, she was diagnosed with severe Malaria and pneumonia. Was admitted for treatment and further investigation. Was given, I.v Artesunate, I.v Dexamethasone , IV 5%D,I.v RL and Ceftroxane. She responded well to medication and recovered gradually and discharged on good condition.
Prognosis
At presentation, Sheryl was clinically unwell, with symptoms suggestive of significant systemic illness due to both severe malaria and pneumonia. Her prolonged fever, inability to retain feeds, and reduced oral intake increased her risk of dehydration and further deterioration. The decision to admit her was appropriate to allow for intensive treatment, fluid management, and observation. Following initiation of therapy, her clinical status is expected to improve steadily.
