Blessings is the youngest child in a family of four siblings. She lives with her single mother and siblings in a modest two-room mud-walled house. She is currently in Grade 9 and dreams of becoming a teacher after completing her education. Her mother is a peasant farmer with no formal employment and relies largely on support from Blessings' older siblings, who perform casual jobs to help meet the family's basic needs. Blessings was previously in good health until she began experiencing unusual fatigue, excessive sleepiness, and becoming easily tired during her normal daily activities. Concerned by these symptoms, she and her mother sought medical attention at a nearby health facility, where she was confirmed to be pregnant. Following the diagnosis, her teenage partner encouraged her to terminate the pregnancy. However, her mother advised against this, citing the potential health risks, and instead provided emotional support and encouraged her to attend regular antenatal care visits. As the pregnancy progressed, Blessings was referred to Siaya County Referral Hospital for specialized obstetric care and monitoring. When she went into labour, she successfully delivered her baby. However, she sustained a perineal tear during childbirth, a complication that required admission to theatre for surgical repair and further management. She remained under medical care for monitoring and recovery following the procedure.

Blessing was diagnosed with a tear after childbirth.,this warranted admission into the theatre for repair. In the theatre saddle block was administered, and a vulval pack left in situ. The tear was repaired and hemotosis confirmed. She was taken to the ward for post operation management, there she was administered pcm, chlorhexidine, plastic, Two ointment among other drugs. Her vitals were monitored, later oral sips was administered. Her catheter and vulval pack removed after 6 hrs. She was eventually discharged on orals and a follow up clinic booked for her

A 4th degree perineal tear is a severe obstetric injury involving disruption of the vaginal wall, perineal muscles, anal sphincter complex, and rectal mucosa. It is not usually immediately life-threatening, but it is a serious condition requiring urgent surgical repair due to the high risk of complications such as severe infection, sepsis, wound breakdown, and long-term fecal incontinence. With prompt and skilled repair, survival is excellent and functional recovery is often good, but delayed or inadequate management can lead to significant lifelong disability affecting bowel control and quality of life.
