Hajara Dau was diagnosed with Antepartum Eclampsia at 34 Weeks Gestation with Fetal Distress. Eclampsia is a severe and life-threatening complication of pregnancy characterized by the occurrence of seizures in a woman with preeclampsia, a condition associated with high blood pressure and organ dysfunction during pregnancy. In this case, the diagnosis was supported by the presence of severe headache, blurred vision, swelling of the face and lower limbs, and multiple generalized seizures. These symptoms indicated significant involvement of the nervous system and severe progression of the disease. The diagnosis was further complicated by Fetal Distress, evidenced by markedly reduced fetal movements, suggesting that the baby was not receiving adequate oxygen and nutrients due to impaired placental blood flow. This placed both the mother and the unborn child at immediate risk and required urgent medical and obstetric intervention to prevent serious complications or death. Antepartum eclampsia is considered a medical emergency and demands prompt treatment to control seizures, stabilize the mother, and ensure the safest possible outcome for both mother and baby.

Hajara is a 24 year old with first pregnancy. Husband is a shoe shiner. They totally earn about 4300 daily.At 34 weeks of gestation,she started experiencing pregnancy related complications. She was rushed to Ayaji Medical and Diagnostic Centre for emergency care. After evaluation, she was diagnosed with Antepartum Eclampsia at 34 Weeks Gestation with Fetal Distress, a life-threatening obstetric emergency that endangers both the mother and the unborn baby. Immediate medical intervention was required to control the seizures, stabilize the mother, and protect the life of the fetus.
Medical history
Treatment and Recovery Upon admission, Hajara Dau was diagnosed with Antepartum Eclampsia at 34 Weeks Gestation with Fetal Distress, a life-threatening obstetric emergency. Immediate treatment was initiated to stabilize both the mother and the unborn baby. She received Magnesium Sulphate to control and prevent further seizures, while Labetalol and Nifedipine were administered to lower her severely elevated blood pressure. Intravenous fluids were carefully given, and she was closely monitored for maternal and fetal wellbeing. Due to fetal distress and the severity of her condition, emergency obstetric management was undertaken to ensure the safest outcome for both mother and child. Recovery Initial Stabilization (First 24–48 Hours): The seizures were successfully controlled, blood pressure gradually improved, and her headache and visual disturbances began to resolve. Continuous monitoring showed progressive stabilization of her condition. Continued Recovery (Days 3–5): Hajara remained seizure-free, her blood pressure became well controlled, and the swelling of her face and legs reduced significantly. Both mother and baby showed encouraging signs of recovery. Discharge and Follow-Up: After achieving clinical stability, she was discharged in good condition with antihypertensive medications, postnatal follow-up appointments, and counseling on future pregnancy care. With timely intervention and close monitoring, both mother and baby had a favorable outcome and prognosis.
Prognosis
Doctor’s Opinion Based on the patient's clinical presentation, including severe headache, blurred vision, generalized tonic-clonic seizures, swelling of the face and lower limbs, and reduced fetal movements at 34 weeks of gestation, the diagnosis of Antepartum Eclampsia with Fetal Distress was established. This is a life-threatening obstetric emergency resulting from severe hypertension in pregnancy, leading to seizures and compromised blood flow to the placenta. The reduced fetal movements indicated that the unborn baby was also at significant risk. Immediate intervention was required to control the seizures, stabilize the mother, and prevent further complications for both mother and child. Prognosis With Prompt Medical Intervention: The prognosis is generally favorable if seizures are rapidly controlled, blood pressure is stabilized, and appropriate obstetric management is provided. Close monitoring and timely delivery can significantly improve outcomes for both mother and baby. Without Immediate Treatment: The prognosis becomes poor, with a high risk of recurrent seizures, stroke, organ failure, placental abruption, fetal distress, stillbirth, or maternal death. Delay in treatment could have resulted in devastating consequences for both the mother and her unborn child. Overall, the patient's condition was critical at presentation but had a good chance of recovery with timely and appropriate medical care.
