Most. Sima Akter is a 32-week-and-5-day pregnant woman, gravida 2, para 1, who presented with a breech presentation, a condition in which the fetus is positioned in the uterus with its buttocks or feet pointing downward instead of the usual head-down position. Due to this complication, the fetus is at risk of developing fetal distress, umbilical cord prolapse, birth trauma such as brachial plexus injury, and head entrapment, which may require immediate medical intervention to ensure the safety of both the mother and the baby.

Background
Most. Sima Akter is a calm and resilient 26-year-old mother who lives with her husband, their 6-year-old son Shahal Hossein, and her elderly mother-in-law in a family of four, doing her best to care for everyone despite their difficult circumstances. Her husband is the family's only earning member, working as a day laborer with a monthly income of approximately BDT 6,000, which is barely enough to provide food and other basic necessities, leaving the family unable to afford the cost of hospitalization, medicines, investigations, and delivery-related care. After learning about Helpster Charity through a community outreach program, the family sought assistance in the hope of ensuring a safe delivery for Sima and her unborn baby. Despite exhausting their limited resources, they cannot continue her treatment without financial support. Without timely medical care and close monitoring, her breech pregnancy may progress to obstructed labour, fetal distress, umbilical cord prolapse, severe maternal bleeding, stillbirth, and other life-threatening complications that could endanger both the mother and her baby.
Medical history
Most. Sima Akter arrived at the hospital at 32 weeks and 5 days of pregnancy with a breech presentation and was admitted on 10.07.2026. She underwent a successful LUCS on the same day, performed by gynecologist Dr. Nahid Akter with anesthesia provided by Dr. Moshiur Rahman. Following the surgery, she received proper medical management, continuous monitoring, and attentive nursing care, which supported her smooth recovery. Both mother and baby remained safe and in stable condition, and she was discharged on 13.07.2026 in good health.
Prognosis
The recommended treatment for Sima Akter is LUCS under SAB, which is considered the safest and most effective option for delivering a baby in a breech presentation. Early surgical intervention can significantly reduce the risk of complications and improve outcomes for both the mother and the baby. With timely treatment, the baby is expected to be delivered safely and recover well. However, if delivery is delayed, serious complications may occur, including fetal distress, umbilical cord prolapse, birth trauma, and head entrapment during labor. Therefore, prompt surgical management is strongly advised to ensure the safety and well-being of both the mother and the child.
