When a case is urgent and vital, hospitals often proceed with delivering medical care right away to save a life, even before the finalisation of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts. That’s why the chronology can sometimes be in disorder.

About infections
Pyelonephritis is a serious bacterial infection of the kidneys, usually caused by bacteria ascending from the bladder (urinary tract infection). Symptoms include high fever, chills, flank or back pain, painful urination, frequent urination, cloudy or foul-smelling urine, nausea, vomiting, and general weakness. If left untreated, pyelonephritis can lead to kidney abscess, sepsis (blood infection), septic shock, permanent kidney scarring, chronic kidney disease, and death. Typhoid fever is a bacterial infection caused by Salmonella typhi, transmitted through contaminated food and water. Symptoms include prolonged high fever, headache, abdominal pain, and weakness. When a child has both conditions simultaneously, the risk of complications increases significantly. Treatment requires hospitalization, intravenous antibiotics (such as ceftriaxone), intravenous fluids, antipyretics, and close monitoring. Prompt treatment is essential to prevent permanent kidney damage and other complications.

Background
Maryam Bashari is a 9-year-old girl living with her elderly grandmother in Birniwa town. She is cared for by her grandmother, who sells firewood to support them, as her parents are absent. Despite their hardship, Maryam is a responsible child who helps after school and remains in school. She presented on 7th June 2026 with a one-week history of high fever, chills, right-sided flank pain, painful urination, urinary frequency, nausea, and vomiting. Her symptoms had worsened despite prior treatment at a local drug shop. On examination, she was febrile and had right costovertebral angle tenderness. Investigations confirmed pyelonephritis and typhoid fever. She was admitted for 4 days for intravenous antibiotics, fluids, and supportive care, and she improved significantly before discharge.
Medical history
Maryam, a nine-year-old girl, was brought to Adari Medical Clinic by her grandmother with a history of fever, flank pain, and painful urination. Following clinical evaluation and laboratory confirmation of pyelonephritis complicated by typhoid fever, she was admitted and treated with intravenous antibiotics, intravenous fluids, and supportive care. Her grandmother was educated on the importance of completing the full course of antibiotics, proper hydration, and recognizing signs of urinary tract infections. At discharge four days later, Maryam showed marked improvement, with resolution of fever, significant reduction in flank pain, and return of appetite. She was sent home with oral antibiotics to complete her treatment and instructions for follow-up. Prognosis
Maryam Bashari needed intravenous antibiotics, intravenous fluids, and close monitoring. She was admitted for 4 days and received intravenous ceftriaxone, intravenous metronidazole, intravenous fluids, and intravenous paracetamol. Her fever resolved by day 2, and her flank pain improved by day 3. At discharge on day 4, she was afebrile, hemodynamically stable, and able to tolerate oral medications. Prognosis is good with prompt treatment. Full recovery expected without long-term kidney damage if follow-up is maintained.
