A complex ovarian cyst is a fluid-filled sac that contains both liquid and solid components, often resulting from hormonal imbalance, endometriosis, or other abnormal ovarian growths. Unlike simple cysts, complex cysts require closer evaluation because they carry a higher risk of complications, including ovarian torsion, rupture, infection, or, in some cases, possible malignant transformation. Patients may present with lower abdominal or pelvic pain, abdominal swelling or bloating, menstrual irregularities, and occasionally pain during movement or intercourse. In some cases, symptoms may worsen over time as the cyst increases in size or becomes complicated. Management usually involves detailed clinical assessment and imaging, with treatment depending on the size, appearance, and symptoms. Surgical intervention is often required, especially when the cyst is large, persistent, or symptomatic, to prevent complications and confirm the diagnosis.
Background
Fatima is a 19-year-old young woman from a low-income household where her parents are petty traders earning less than ₦25000 per month. Despite the family's financial challenges, she is hardworking, responsible, and actively supports her parents with household chores and small trading activities. She is friendly and sociable, enjoys spending time with her friends, and is well regarded within her community. Like many adolescents her age, she hopes for a brighter future and the opportunity to pursue her goals. However, the family's limited financial resources have significantly affected their ability to access timely and adequate healthcare, making it difficult to meet the cost of essential medical treatment. Recently, her symptoms worsened significantly, causing persistent lower abdominal pain that interfered with her sleep and prevented her from carrying out her usual daily activities. Her parents learned about Helpster Charity Foundation through a previous beneficiary whose child had successfully undergone hernia surgery two years earlier. She subsequently presented to UMD with complaints of lower abdominal swelling, pain, and irregular menstrual cycles. Following evaluation, she was diagnosed with a complex ovarian cyst requiring surgical removal and has been scheduled for surgery upon approval by Helpster Charity Foundation.
Prognosis
Fatima was diagnosed with a complex ovarian cyst causing significant discomfort. Based on this assessment, surgical management is the most appropriate option to prevent potential complications such as cyst rupture, infection, and pelvic abscess formation. With prompt and appropriate treatment, she is expected to have a favourable outcome, with relief of symptoms and full recovery.
