Bartholomew is a 16-year-old boy in Grade 10 and the fourth-born in a family of six children. He lives with his family in a rural village and is known to be respectful, hardworking, and determined. He enjoys playing football, reading, and helping his mother with farming and household chores. His family depends on his mother, a small-scale farmer who earns about KSh 1,000 per month from selling green groceries, making it difficult to meet their basic needs. Bartholomew developed severe tooth pain that rapidly progressed to high fever, marked swelling beneath the jaw, severe facial and neck pain, difficulty swallowing, and inability to open his mouth properly. As the swelling worsened, his tongue became enlarged, making it difficult for him to speak and placing him at risk of airway obstruction. Alarmed by his deteriorating condition, his grandmother rushed him to the health facility. Due to the family's financial hardship, they could not afford treatment. A Community Health Promoter identified the case, recognized the medical emergency, and referred Bartholomew to Helpster Charity, enabling him to access urgent lifesaving care.

Bartholomew was admitted to Bulondo Health Centre with Ludwig’s angina, a severe bacterial infection affecting the tissues beneath the tongue and floor of the mouth that caused significant swelling, pain, difficulty swallowing, and risk of airway obstruction. Due to the seriousness of his condition, he was immediately assessed and admitted for urgent treatment. Through the support of Helpster Charity Organization, his family was able to access lifesaving medical care without the burden of hospital expenses.During his admission, Bartholomew received intravenous antibiotics to control the infection, intravenous fluids to maintain hydration, pain relief medication, and close monitoring of his airway and vital signs. The medical team also provided supportive nursing care, nutritional support, and surgical management where necessary to drain the infection and reduce the swelling.Over the course of treatment, Bartholomew responded well. The swelling gradually reduced, his pain subsided, and he regained the ability to swallow, eat, and speak comfortably. His infection resolved without serious complications, and his overall condition improved steadily.After successful treatment and stabilization, Bartholomew was discharged in a stable condition with oral medications and follow-up instructions to ensure complete recovery. His family expressed heartfelt appreciation to Helpster Charity Organization and Bulondo Health Centre for their timely, compassionate, and lifesaving support, which enabled him to recover and return to his normal daily activities.

**Doctor's Opinion and Prognosis** Bartholomew has been diagnosed with Ludwig's angina, a rapidly progressing and potentially life-threatening bacterial infection of the tissues beneath the tongue and jaw. The extensive swelling places him at immediate risk of airway obstruction, severe sepsis, and spread of infection to the chest if not treated promptly. He has been admitted for urgent management with intravenous antibiotics, fluids, pain control, and close airway monitoring, with surgical drainage if required. His prognosis is cautiously favorable provided treatment is initiated without delay and he responds well to therapy. Any delay in management could result in life-threatening complications.
