Typhoid fever is a serious bacterial infection caused by Salmonella Typhi, usually spread through contaminated food or water. It commonly causes persistent high fever, abdominal pain, diarrhea or constipation, headache, weakness, poor appetite, and vomiting. If left untreated, it can lead to intestinal bleeding or perforation, bloodstream infection, and other life-threatening complications. Prompt treatment with appropriate antibiotics and adequate hydration is essential. Severe pneumonia is a life-threatening lung infection in which the lungs become inflamed and filled with fluid or pus, making breathing difficult and reducing oxygen supply to the body. Symptoms include high fever, persistent cough, rapid or difficult breathing, chest indrawing, low oxygen levels, weakness, and poor feeding. It requires urgent hospital treatment to prevent respiratory failure, sepsis, and death.

Background

Alex is a 13-year-old boy and the third-born in a family of five children. He lives with his parents in a remote village where his father earns about KSh 1,000 per month repairing bicycles while his mother is a homemaker. The family has no electricity and walks over 30 minutes to fetch water from a well. Despite these challenges, Alex is hardworking, enjoys reading storybooks and playing football, helps his father repair bicycles after school, and dreams of becoming a lawyer. Three days before admission, Alex developed persistent diarrhea, severe headache, abdominal pain, chest congestion, difficulty in breathing, and progressively rapid breathing. His mother initially treated him with over-the-counter medicines, but his condition worsened, leaving him weak, breathless, and unable to carry out his normal activities. At the health facility, clinical assessment and investigations confirmed typhoid fever and severe pneumonia. His low oxygen saturation (89%) indicated significant respiratory compromise, requiring urgent admission, nebulization, close monitoring, and inpatient treatment. Because his family could not afford the required medical care, Alex was referred to Helpster Charity for financial assistance.

Medical history

Alex was admitted to the hospital after experiencing a high fever, persistent cough, chest pain, difficulty breathing, weakness, and stomach discomfort. After medical examination and laboratory tests, doctors diagnosed him with typhoid fever and severe pneumonia. He was admitted for close monitoring because both illnesses required prompt treatment. During treatment, Alex received intravenous (IV) antibiotics to fight the infections, IV fluids to prevent dehydration, oxygen therapy to improve his breathing, and medications to reduce fever and relieve pain. He was also encouraged to rest, eat a balanced diet, and drink enough fluids to support his recovery while healthcare providers monitored his condition regularly. After several days of treatment, Alex's condition improved significantly. His fever subsided, his breathing became easier, and his strength gradually returned. He was discharged from the hospital with oral medications, advice to complete the prescribed treatment, maintain good hygiene, eat nutritious foods, and attend follow-up appointments to ensure a full rec

Prognosis

Alex presented with severe pneumonia complicated by typhoid fever, resulting in significant respiratory distress and systemic infection. His low oxygen saturation indicated that he required urgent inpatient care, including oxygen support, antibiotics, nebulization, intravenous fluids, and close monitoring. Delayed treatment could have resulted in respiratory failure, severe sepsis, or other life-threatening complications. Alex is responding positively to treatment, and his condition has continued to improve.
