Fetal distress refers to signs that the baby is not receiving enough oxygen before or during labour, often shown by an abnormal fetal heart rate. It requires urgent medical intervention to prevent serious injury or death. A complete breech presentation occurs when the baby is positioned with the buttocks or feet closest to the birth canal instead of the head. In a woman with a previous Caesarean section scar, this greatly increases the risk of difficult delivery, uterine scar rupture, and injury to both mother and baby during labour.

Background
Purity is the third-born in a family of seven children. She lost her mother at a young age and, due to financial hardship, dropped out of school in Form Three, as did several of her siblings. She lives with her father, a peasant farmer, in a modest two-room mud-walled house and earns a living through small-scale farming. Despite the challenges she faces, Purity remains hopeful about building a better future for herself and her child. Purity suspected she was pregnant after missing her menstrual periods and developing dizziness, persistent vomiting, and fatigue. Pregnancy was confirmed at a nearby dispensary, after which she enrolled in antenatal care and attended all her scheduled ANC visits. Towards the end of the pregnancy, she developed progressively worsening lower abdominal pain, abdominal cramps, severe back pain, restlessness, and regular labour pains, prompting her to seek urgent medical attention. She was admitted to Siaya County Referral Hospital, where assessment revealed a previous Caesarean section scar, a complete breech presentation, and signs of fetal distress. An emergency Caesarean section was therefore performed to safely deliver the baby and prevent life-threatening complications for both mother and child.
Medical history
Purity presented to the maternity unit.In active phase of labour with complains of regular lower abdominal pains.Supportive care was initiated including monitoring of viital signs,fetal heart rate and labour progress.Intravenous fluids were administered,iv flagyl,pcm,among others.She was treated and discharged on stable condition.Clinic booked for follow up.
Prognosis
Purity has been diagnosed with a complete breech presentation in the presence of a previous Caesarean section scar, accompanied by fetal distress. This is an obstetric emergency, and continuing labour would place both mother and baby at significant risk of uterine rupture, severe bleeding, birth trauma, oxygen deprivation, or death. An emergency Caesarean section has therefore been recommended as the safest mode of delivery. If the operation is performed promptly and appropriate postoperative care is provided, the prognosis for both mother and baby is expected to be favorable.
