Severe post-burn flexure contracture of the left knee is a disabling condition caused by scar tissue shortening following a burn injury. It results in persistent knee flexion, restricted extension, impaired walking, reduced mobility, difficulty standing, and limitations in daily activities. Long-term rehabilitation and possible surgical contracture release may be required.

Background

Yinang is a 13yo old male child who has not been in school because of his condition, he has 7 other siblings. The father farms to provide for the family and the mother is a house wife. They sometimes struggle to get what to eat. They live in a village where the house has 3 separate rooms with a pit toilet. Water is from the well about 20mins apart to the well. No electricity and monthly income is 5000. Yinang sustained injury from fire and he was managed by a traditional healer, the site got infected and started draining pus around his knee leading to ulceration. When the wound healed, his leg was attached to the thigh. Patient has not been able to walk with the affected leg. No other medical illness. Patient is a primary school pupil who is currently out of school due to inability to walk to school. Yinang is not on any form of distress, not pale. There is severe contracture of the left knee at the popliteal region. The muscles of the leg and thigh have atrophied and shortened but the knee joint is pliable. Unstable scar noted at the popliteal fossa and medial as well as lateral aspect of the thigh/knee. Diagnosis: Severe Postburn Left Knee Flexure Contracture.

Prognosis

Clinical examination confirms a severe post-burn flexure contracture of the left knee with significant functional impairment for Yinang. Prognosis is guarded, requiring intensive rehabilitation and possible surgical release to improve mobility and function.
