Why is treatment sometimes done earlier than the payment?

When a case is urgent and vital, hospitals often proceed with delivering medical care right away to save a life, even before the finalisation of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts. That’s why the chronology can sometimes be in disorder.

Post-burns contracture is a deformity that results from abnormal healing of burns injuries. If it occurs across joints like in this case, it impairs the function of such joints and render such body part useless or inefficient. If not treated on time, it gets the joint stiff and impair the growth of the body part in growing children leading to discrepancies in growth.

Background

Eight-year-old Rukayya is a happy child despite carrying scars from infancy. When she was a baby, her room caught fire, leaving her with severe flame burns; her mother, who was with her, was also burned on her face and body. Initial treatment by a nonspecialist left Rukayya with severe contractures on both upper limbs, and she has since endured three surgeries. Her family, peasant farmers in a remote village, have stood by her through it all, selling their properties and even their land to pay for her care. Now, with nothing left, they have been unable to afford her next surgery. Rukayya loves playing with other children, though shame over her deformed hands once made her isolate herself. Yet from her long hospital stays, something beautiful grew: a love for healthcare. Rukayya dreams of becoming a medical doctor someday, and she wishes for her hands to be treated and fully functional so she can study and reach that dream.

Prognosis

Without further surgery, the contractures will keep tightening as Rukayya grows, since scar tissue cannot stretch with her body. Her hands and arms will become progressively more deformed and less functional, potentially fusing joints permanently and undoing the gains of her previous surgeries. She faces lifelong disability, limiting her ability to write, learn, and care for herself, along with deepening social isolation. Timely surgery now can restore function and keep her dream of becoming a doctor alive.
