About pulmonology
Severe pneumonia is a serious lung infection causing inflammation and fluid in the alveoli. It is life-threatening because it prevents the lungs from absorbing enough oxygen, putting vital organs at risk. It requires immediate emergency medical attention. Bronchiolitis is a common, contagious lung infection that causes inflammation and congestion in the small airways (bronchioles) of the lung. It primarily affects babies and children under 2 years old and is usually caused by a viral infection, most commonly the Respiratory Syncytial Virus (RSV).

Background
Elizabeth is the second-born in a family of two children. She lives with both parents and her sibling in a single-room mud-walled house with an iron-sheet roof. Both parents are peasant farmers who depend on small-scale farming to provide for the family. Elizabeth is a cheerful young child who enjoys playing with toys. Due to her age, she has not yet developed specific hobbies or future career aspirations. On 29/06/2026, Elizabeth suddenly became seriously ill with a high fever, persistent cough, repeated vomiting, excessive sweating, and inconsolable crying. Her condition rapidly worsened, leaving her weak, irritable, unable to feed well, and requiring urgent medical attention. She was initially taken to Barding Dispensary, where she received emergency assessment and basic investigations before being referred to Siaya County Referral Hospital for specialized care. At the referral hospital, she was evaluated in the casualty department and admitted to the paediatric ward for further investigations, close monitoring, and appropriate treatment.
Medical history
Elizabeth was brought to the hospital with complaints of coughing and noisy breathing. She was diagnosed with severe pneumonia and bronchiolitis, which warranted admission. In the ward she was administered pcm, she was nebulised with salbutamol 6 hourly, dexamethasone among other drugs. She responded well to medication and was eventually discharged on orals and a follow up clinic booked for her.
Prognosis
Elizabeth a 2-year-old child was admitted at Siaya County Referral Hospital with a diagnosis of severe pneumonia and bronchiolitis. The prognosis was initially guarded due to the severity of the lower respiratory tract infection, which posed a risk of respiratory distress, hypoxia, and other complications if not promptly managed. Following admission, the child received appropriate inpatient care, including oxygen therapy, supportive treatment, adequate hydration, and close clinical monitoring. With this timely and appropriate management, the prognosis is generally favorable, and she is expected to recover fully without long-term complications.
