When a case is urgent and vital, hospitals often proceed with delivering medical care right away to save a life, even before the finalisation of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts. That’s why the chronology can sometimes be in disorder.

Severe acute malnutrition (SAM) is a life-threatening condition defined by severe wasting (very low weight-for-height), nutritional edema (swelling of both feet), or a mid-upper arm circumference (MUAC) under 115 mm. It severely compromises immune function, significantly increasing the risk of mortality in young children. Septicemia is the clinical term for blood poisoning. It is a life-threatening condition where bacteria, viruses, or fungi enter the bloodstream and multiply, releasing toxins that spread throughout the body.

Background
Loren is the youngest of five children. He lost his father a few years ago following a short illness, leaving his mother as the sole provider for the family. She works as a househelp, but her income is insufficient to meet the family's basic needs. Due to financial hardship, the family relocated to their maternal grandmother's home, where they live in a one-room mud-walled house with an iron-sheet roof. Loren's grandmother, a peasant farmer, also helps support the household. As a young child, Loren enjoys playing and has not yet developed specific hobbies or future career aspirations. Loren developed a high fever, repeated vomiting, diarrhoea, swelling of the legs, marked fatigue, poor appetite, and progressive weight loss. His condition worsened, leaving him weak and less active. He was initially taken to Kabura Dispensary, where he received first aid treatment, including paracetamol, before being urgently referred to Siaya County Referral Hospital. He was admitted through the casualty department and later transferred to the paediatric ward, where investigations confirmed severe acute malnutrition and septicaemia. He is currently receiving comprehensive treatment, nutritional rehabilitation, and close medical monitoring.
Medical history
Loreen was brought to the hospital with complaints of poor feeding, degreaser milestone. He was diagnosed with severe acute malnutrition and septicemia which warranted admission. In the ward he was administered ceftriaxone,pcm among other drugs. He was also administered ringers lactate day and night to which he was tolerant. His mother was nutritionally counselled, and he was eventually discharged on orals with a nutritional clinic booked for them in a health facility near their home.
Prognosis
A 1-year-old with septicemia and severe acute malnutrition has a serious but potentially reversible condition. With early diagnosis, aggressive medical treatment, and nutritional rehabilitation in a hospital setting, many children recover. However, because both conditions are individually life-threatening and together significantly increase the risk of complications, the child requires urgent inpatient management and close follow-up after discharge.
