Viriam A., 15

Viriam is a 15-year-old girl, the firstborn in a family of four children. She lives with her parents and siblings in a two-room mud-walled house and is in Grade 8, attending school through bursary support. Her mother is a peasant farmer while her father survives on casual jobs, and together they struggle to meet the family's basic needs. Viriam enjoys reading and dreams of becoming a doctor. During a school break, her grandmother noticed she was unusually tired and suspected she might be pregnant after seeing her with a fellow teenager. Her mother took her to a health facility, where tests confirmed a four-month pregnancy. Despite the unexpected pregnancy, Viriam continued her education and attended all four recommended antenatal visits. Near term, she developed abdominal pain and was referred to Siaya County Referral Hospital after being found to have a breech presentation. She delivered vaginally but sustained a vaginal tear that required treatment and follow-up. After delivery, her family was unable to pay the hospital bill until they were informed about Helpster Charity, which had previously supported another teenage mother in a similar situation.

Variam was diagnosed with a tear after childbirth.,this warranted admission into the theatre for repair. In the theatre saddle block was administered, and a vulval pack left in situ. The tear was repaired and hemotosis confirmed. She was taken to the ward for post operation management, there she was administered pcm, chlorhexidine, plasil among other drugs. Her vitals were monitored, later oral sips was administered. Her catheter and vulval pack removed after 6 hrs. She was eventually discharged on orals and a follow up clinic booked for her

A 4th degree perineal tear is a severe obstetric injury involving disruption of the vaginal wall, perineal muscles, anal sphincter complex, and rectal mucosa. It is not usually immediately life-threatening, but it is a serious condition requiring urgent surgical repair due to the high risk of complications such as severe infection, sepsis, wound breakdown, and long-term fecal incontinence. With prompt and skilled repair, survival is excellent and functional recovery is often good, but delayed or inadequate management can lead to significant lifelong disability affecting bowel control and quality of life.
