Why is treatment sometimes done earlier than the payment?
When a case is urgent and vital, hospitals often proceed with delivering medical care right away to save a life, even before the finalisation of admission procedures. Usually, when this happens, hospitals put the bill on our credit until the funds are transferred to their accounts. That’s why the chronology can sometimes be in disorder.

About infections
Osteomyelitis is a serious infection of the bone, usually caused by bacteria. It may cause severe persistent pain, swelling, warmth, fever and a chronic wound that may discharge pus. If not adequately treated, the infection can damage or destroy bone, spread to surrounding tissues or bloodstream, and cause long-term disability.

Background
Shadrack is a 17-year-old Grade Nine student and the third-born in a family of seven children. He lives with his parents, siblings and some extended family members in a temporary home after the family was displaced following a land dispute. Their limited income must cover food, education and other basic needs, leaving little for healthcare. Shadrack enjoys volleyball and watching football and hopes to become a teacher. For nearly four years, he has suffered from a recurrent wound on his leg that repeatedly improves and worsens. Shadrack was brought to the facility by his mother after the wound deteriorated significantly. He was in severe pain and unable to walk normally, having to protect the affected leg with every step. The wound was visibly infected, with pus oozing from the affected area, associated with swelling and significant tenderness. The persistent pain had interfered with his sleep, movement, school attendance and usual activities. He appeared exhausted and distressed from the prolonged illness. Following assessment, the doctor determined that the severity and chronic nature of the infection required hospital admission.
Medical history
Achapa spent days in the ward being managed with high antibiotics and daily cleaning of the word. Later, he was referred to Inuka Hospital in Kisumu for further re-assessment and management
Prognosis
Shadrack was diagnosed with osteomyelitis after presenting with a longstanding, painful and pus-discharging leg wound that had significantly affected his walking and daily activities. The chronic nature of the infection requires appropriate antibiotics, wound care, pain management and surgical assessment where indicated. Without adequate treatment, osteomyelitis may cause permanent bone damage or recurrent infection. With timely, prolonged treatment and proper follow-up, his prognosis is cautiously favourable, although recovery may take time.
